According to the Transportation Security Administration (TSA) of the U.S.  Department of Homeland Security Regulation,Passengers to the United States before taking a flight to the United States,Must be filled out in advance”COVID-19 Proof of disease declaration form ”.Passengers must log on “HERE website”in advance to check the specific requirements, download, print and complete the declaration form in  ”PROOF OF COVID-19 VACCINATION FOR NONCITIZEN NONIMMIGRANTS

PASSENGER DISCLOSURE AND ATTESTATION TO THE UNITED STATES OF AMERICA ”,and provide it to the airlines staff for retention.If a passenger to the United States fails to fulfill the information provided in the above form, the relevant U.S. government authorities will not allow the airline to accept the passenger boarding the flight.
